A 36-year-old otherwise healthy male with a history of multiple recent sexual partners presented with annular erythematous plaques on the lower legs and hyperpigmented palmoplantar macules with a peripheral collarette of scales suggestive of Biett's sign. Secondary syphilis was confirmed by a VDRL titer of 1:8 and reactive TPHA. This case highlights the diagnostic value of recognizing Biett's sign, as a characteristic clue to secondary syphilis.
