Product quality management is a critical issue for manufacturing enterprises in the context of increasing market competition and customer expectations. In the carton packaging industry, production defects lead to material waste, increased costs, reduced operational efficiency, and lower customer satisfaction. Therefore, identifying the root causes of defects and implementing effective quality improvement solutions are essential for sustainable business development. This study investigates quality defects in the carton packaging production process at Tan Long Paper Packaging Company Limited, focusing on identifying major defects and proposing appropriate improvement measures. The research particularly examines the two most critical defects: warping and torn crease.The study employed both qualitative and quantitative approaches through direct observation, production data collection, expert surveys, and statistical analysis. The Failure Modes and Effects Analysis (FMEA) method was applied to evaluate and rank defects using the Risk Priority Number (RPN). In addition, Pareto charts and fishbone diagrams were used to identify priority problems and analyze root causes.The findings indicate that warping and torn crease defects caused the highest production losses and risk levels. After implementing improvement solutions, the RPN of warping defects decreased from 448 to 160, while torn crease defects decreased from 392 to 140. The study concludes that integrating FMEA with root-cause analysis effectively improves product quality, reduces production costs, and enhances operational efficiency.
