Abstract Adenomatoid odontogenic tumor (AOT) is an uncommon odontogenic tumor with unique clinicopathological features and a debatable etiology as to whether it is a hamartoma or a neoplasm. It is characterized histologically by spindle or cuboidal epithelial cells forming duct-like structures, eosinophilic amyloid, and calcifications. AOT having dilated blood vessels of varying sizes is rarely described in the published literature, although other odontogenic tumors like hemangiomatous ameloblastoma are recognized. It is interesting to debate whether it is a hemangiomatous variant or a collision tumor. This article describes the case of a 20-year old female presenting with a maxillary AOT with numerous dilated blood capillaries. Due to the scarcity of such lesions, their clinical behavior is yet to be ascertained, although bleeding can be encountered during surgical excision. Hence, awareness among clinicians is essential.
