Miyastenia gravis (MG), klinik gidişatı büyük ölçüde otoantikor profiliyle şekillenen otoimmün bir hastalıktır. Özellikle ileri yaş başlangıçlı olgularda ve timoma varlığında saptanan anti-titin antikorlarının, hastalık şiddeti ve kötü prognozla doğrudan ilişkili olduğu bilinmektedir. Bu olgu serisinde, geleneksel tanı testlerinin yetersiz kalabildiği atipik paraneoplastik MG vakalarında anti-titin antikorlarının tanısal değeri ve klinik yönetimdeki kritik rolüne dikkat çekilmesi amaçlanmıştır.
