Ameloblastoma is a benign odontogenic neoplasm with locally aggressive behavior and radiographic features that may mimic odontogenic cystic lesions. This case report describes a conventional ameloblastoma in a 16-year-old male patient involving the right posterior mandible and associated with an impacted mandibular third molar and an accompanying paramolar tooth. The lesion exhibited clinical and radiographic features mimicking a dentigerous cyst. Panoramic radiography and cone-beam computed tomography revealed a well-defined expansile radiolucent lesion with cortical thinning, focal perforation, displacement of the impacted tooth, and inferior displacement of the mandibular canal. After incisional biopsy, histopathological examination confirmed conventional ameloblastoma, plexiform subtype. Conservative treatment was performed using decompression with an acrylic obturator. During the three-year clinical and radiographic follow-up, substantial bone regeneration was observed, and no recurrence was detected. This case highlights the need to consider conventional ameloblastoma in the differential diagnosis of cyst-like lesions associated with impacted teeth in young patients.
