Background: Coronary artery anomalies (CAAs) represent one of the most complex pathologies in cardiology. Right coronary artery anomaly (RCA) is extremely rare. Often this condition is asymptomatic and diagnosed accidentally; however, in some cases it can have a catastrophic onset and cause sudden death. Case Report: We present here a rare case of sudden cardiac death (SCD) of a 27-year-old man due to a RCA with origin from the left coronary sinus and abnormal course, exacerbated by the Valsalva manoeuvre during defaecation. Conclusion: We emphasise the need for a careful and comprehensive approach in cases of SCD. In these cases, a careful and thorough analysis of the origin and course of the coronary arteries is necessary.
