Background: Lateral epicondylitis is a common degenerative tendinopathy affecting the common extensor origin of the elbow. Corticosteroid injections provide rapid pain relief whereas; platelet rich plasma (PRP) has emerged as a regenerative biological therapy. Methods: A prospective comparative study was conducted among 72 patients with chronic lateral epicondylitis in a tertiary care centre at Puducherry between March 2024 and September 2025. Participants were randomly allocated into two groups: group A corticosteroid injection (n=36) and group B PRP injection (n=36). Outcomes were assessed using Visual Analogue Scale (VAS) and Mayo Elbow performance score (MEPS) at baseline, 1 week, 2 months and 6 months. Results: Both groups demonstrated significant improvement in pain and function following intervention. Corticosteroid injections produced earlier symptomatic relief, whereas PRP demonstrated superior long term functional recovery and sustained pain reduction at 6 months. Complications such as skin depigmentation and subcutaneous atrophy were more frequent in the steroid group. Conclusion: PRP is an effective and safe treatment modality for chronic lateral epicondylitis and provides superior long term functional outcomes compared to corticosteroid injections.
