Abstract Large periapical lesions are common complications of pulp necrosis and may delay healing after standard root canal treatments. While surgical endodontic techniques can facilitate healing, they often involve more invasive procedures and are less accepted by patients. To address these issues, a simplified through-the-canal method was developed for the minimally invasive management of periapical lesions. This case series aimed to evaluate the clinical feasibility, safety, and patient comfort of a novel micro-debulking device used for managing periapical lesions. Three patients diagnosed with symptomatic apical periodontitis and a periapical index (PAI) score of 2 or higher were treated using a novel device following conventional chemomechanical preparation of the incisors and first premolar. The outcomes assessed included postoperative pain reduction measured by the Visual Analog Scale (VAS), the feasibility of the procedure, and radiographic healing evaluated through PAI scores at a 3-month follow-up. Results showed favorable symptomatic relief and improved radiographic healing in all cases, with no serious complications reported.
